Do you need to make an appointment? Please call us at 320-632-1950 or complete the form on the “Appointments” tab; then see the steps below.
Section One
If you are a patient with a brain injury/or other neurological condition, have had a concussion, whiplash injury or a car accident, please complete these steps (if you are a patient not related to brain injury/or any of these conditions – please scroll down and see below):
Step 1. Watch the Introduction to Neuro-Optometry video. This is a requirement in order to complete the next steps.
Step 2. Complete the Neuro-Optometry Health History form HERE, at least 1 week prior to your first appointment.
**Please note: you must complete Steps 1 and 2 in order to be seen for your first appointment!** If you need help completing, please call Thrive With Vision.
Step 3. Please see the “Contact” tab for the address and a map to Thrive With Vision.
We will send you a text reminder prior to each appointment. **Please note: there is a fee if you miss the appointment or cancel it without 24 hours notice.**

Section Two
If you are a patient, not related to a brain injury/or other neurological condition, please complete these steps:
Step 1. Complete the Eye Care Health History form HERE at least 3 days prior to your appointment.
**Please note: you must complete Step 1 in order to be seen for your appointment!** If you need help completing, please call Thrive With Vision. OR, please arrive 30 minutes prior to the appointment time and complete the form in the office.
