Do you need to make an appointment? Please call us at 320-632-1950 or complete the form on the “Appointments” tab; then see the steps below.
Section One
If you are a patient, not related to a brain injury/or other neurological condition, please complete the first part of the patient information through social history section of the health history. You do not need to fill out the specialty clinic information listed after the ‘STOP’.
If you are a patient with brain injury/or other neurological condition, have had a concussion, whiplash or a car accident you will need to fill out both sections of the health history. You will check yes when prompted and will continue to fill out the specialty clinic information. It is required to watch the Neuro-Optometry Video prior to your appointment.
Step 1. Complete the Eye Care Health History form HERE at least 1 week prior to your appointment.
Step 2. Please see the “Contact” tab for the address and a map to Thrive With Vision.
**Please note: you must complete Step 1 in order to be seen for your appointment!** If you need help completing, please call Thrive With Vision 320-632-1950.
Section Two
Step 1. If you have already been seen by Dr. Miller for an initial assessment following a brain injury, concussion, whiplash injury or other neurological concerns, please fill out the follow-up health history. This needs to be filled out each time you are seen by Dr. Miller if it has been over 1 month since your last appointment.
Step 2. Complete the Neuro-Optometry Health History follow up form HERE, at least 1 week prior to your first appointment.
We will send you a text reminder prior to your appointment. **Please note: there is a fee if you miss the appointment or cancel it without 24 hours notice.**
